YOUR COST FOR SERVICES:

Dog boarding and Cat boarding and kitten

Your furry friend will receive daily attention from us to make the visit enjoyable and as stress free as possible. Indoors they will have music. **Doggie friends will also enjoy romping and rolling on the grass in the large yard (frequently every day).

BY APPOINTMENT ONLY. Sorry, WALK-INS ARE NOT OK.

Discounts are available for more than one pet, extended stays, and/or if paying cash.

DOGS ****Costs are for 24 hours and determined by size

Small (0-15 lbs) $23 Medium (16-35 lbs) $24

Large (36- 55 lbs) $25 X-large (56 lbs and over) $26

CATS & KITTENS****Cost is for 24 hours… $24

OTHER PETS: Call for cost and availability for other pets.

There is a $30.00 deposit for all pets (refunded when your pet is picked up). Full payment is due at time of drop off.

NOTE: Things you will need to bring for your pet’s stay:

1. Proof from your veterinarian of current vaccinations.

***Dogs: Rabies, Bordetella and DHLP

***Cats: Rabies and PLP

By requiring proof of current vaccinations for your pet, we are able to minimize the spread of disease from one pet to another.

2. Pet food and treats

Pets are happier and healthier when their food and treats stay consistent. For this reason, you are required to provide the food and treats that your pet is already accustomed to. This also prevents digestive upsets while in our care.

3. Any favorite toys

Pets feel more at ease and have less anxiety about staying in a new place when they have their favorite toys with them to play with.

4. Favorite blanket or bed

Pets understand that they have their own sleeping and resting space when they have access to a bed or blanket that they are already accustomed to from their home. They sleep better and feel safer.

5. Any other items that will make your pet feel more at home.

If there is an item that your pet is attached to, feel free to bring it for the comfort and happiness of your furry friend.

6. Medications

Medications will be given at times that you designate and following your directions. It is important to have a consistent medication regimen.

7. Leash and collar

Please bring your pet with a fitting collar and leash on them at the time of your arrival.

Another NOTE: Things to NOT bring: Fleas or ticks.

Please check your pet for fleas and ticks before your visit. Please use your flea prevention method prior to bringing your pet. Pets with fleas or ticks will not be allowed to stay.

BOARDING/DAYCARE FEES

________ I understand that if I fail to pick up my pet by above pickup time, I agree to pay an additional $5.00 per hour that I am late and that I may not pickup my pet late without calling or texting for appointment time.

______________ I understand that if I fail to pick up my pet by 6:00 PM, my pet will be checked into boarding for the night and will be available for pickup the following day by a prearranged appointment time. I agree to pay associated boarding fees and a late pickup fee of $20.00.

_____I agree/consent to Awesome Pets LLC keeping my pet and refusing to release my pet until I have paid all charges due to the facility. The facility is hereby granted a lien on my pet for any and all unpaid charges resulting from daycare, boarding, damage to the facility, or any other service provided by the facility.

HEALTH & WELLNESS

_____ I agree to provide proof of required vaccinations to Awesome Pets LLC

_____ In the event that my pet displays symptoms of being unwell (e.g. unusual tiredness, vomiting, diarrhea, coughing, discharge from eyes or nose, hair loss, or etc.), I agree to NOT bring my pet to daycare/boarding without prior approval from the facility.

_____ I agree in the event that another pet in my home is unwell or has been diagnosed with a contagious illness, not to bring my pet for daycare/boarding

_____I give my permission/consent for Awesome Pets LLC to act in the owner’s behalf to obtain veterinary services if my pet becomes ill or requires veterinarian services, administer medicine, or give other requisite attention to my pet; that all expenses incurred shall be paid by me, and indemnify and hold Awesome Pets LLC and it’s employees harmless from said expenses. I will pay the full expenses associated with treatment when I pick up my pet. I understand that an attempt will be made to contact my veterinarian, but I give consent to use an alternative veterinarian of the facility’s choice and discretion.

_____I understand that I am responsible for keeping my pet on a flea treatment program and will verify that my pet is free from fleas prior to bringing it to Awesome Pets LLC facility.

_____I understand that my pet may have sore muscles, sore joints, and fatigue after a day of daycare or boarding.

____In the event that my pet displays recent aggression towards other pets and/or people, I will notify Awesome Pets LLC.

Further terms and agreements

_____I understand that Awesome Pets LLC is not responsible for any belongings that are left for my pet.

_____ I understand that I am responsible for any property damage caused by my pet and agree to reimburse Awesome Pets LLC at time of pick-up of my pet.

_____ I understand that if I am late picking up my pet for three (24 hr) days and have not made arrangement for pick-up and payment of late fees, my pet will be considered to be abandoned. Abandoned pets will be taken to an adoption facility for re-homing.

_____I assume any and all liability and expenses for injuries inflicted on any human or other pets by my pet while in the facility. I agree to hold Awesome Pets LLC and it’s employees harmless from any and all liability and expenses incurred as a result of said injuries.

_____I give Awesome Pets LLC permission/right to take photographs and/or videos of my pet while in the facility. The facility may post or reproduce all photographs and/or videos taken (as well as pet(s) name) on or in, without limitations to facility’s website, social media sites, or promotional materials without becoming liable to me (the owner) or to my pet for any royalty payment. Owner hereby releases, covenants not to sue, and forever discharges the facility of and from any and all claims, demands, rights, and causes of action of whatever kind or nature. Further, I, the owner agrees that no photography or video may be taken by the owner while in the facility without written approval from Awesome Pets LLC.

_____I, the owner, agree that Awesome Pets LLC’s liability for anything to do with my pet, shall not exceed the current chattel value of my pet of the same species, age, and condition delivered to the facility.

_____I, the pet owner, releases, indemnifies, and holds the facility, Awesome Pets LLC, and it’s employees harmless from all manner of damages, claims, losses, liabilities, costs, or expenses, causes of action or suits, whatsoever in law or equity (including without limitation, attorney’s fees and related costs) arising out of or related to the services provided by the facility.

_____Any controversy or claim relating to this agreement/contract shall be settled by arbitration in accordance with the rules of the American Arbitration Association. Judgement upon the award rendered by an arbitrator may be entered in any court having jurisdiction thereof. The arbitrator shall as part of his/her award, determine an award to the prevailing party of the costs of such arbitration and reasonable attorney’s fees of the prevailing party.

_____This agreement/contract contains the entire agreement between Awesome Pets LLC and the pet owner and shall be binding on the heirs, administrators, personal representatives, and assigns of the owner and the facility.

_____This is an agreement/contract between Awesome Pets LLC (also referred to as “facility) and the pet(s) owner whose signature appears below.

ANIMAL BOARDING AND ACKNOWLEDGMENT OF RISK

In consideration of the services provided by Awesome Pets LLC, its officers, agents, and employees and all other persons or entities associated with the facility, I agree as follows:

I understand that while my pet is staying at Awesome Pets LLC, he or she may come in contact with other pets and humans. Every effort will be made to ensure the safety of pets, but there are inherent risks involved with interactions between humans and lets, as well as between one pet and another pet, which may result in property damage, bodily injury, including but not limited to sickness or death; and there may be other risks not known to me or to Awesome Pets LLC that are not readily foreseeable at this time. I understand that there are certain risks inherent in boarding my pet that cannot be eliminated. Awesome Pets LLC believes it is important for me to know in advance what to expect and the potential risks, and that unknown risks may exist. In the event of an injury or illness, we will make every effort to contact you. If we are unable to reach you, your pet will be taken to our local veterinarian clinic.

I fully accept responsibility for these inherent risks (which may or may not be specifically identified herein). My use of Awesome Pets LLC is purely voluntary, and I elect to proceed hereunder in spite of and with full knowledge of the risks and possibilities of unknown risks.

I have fully read, and understand and accept, the terms conditions stated herein and acknowledge that this agreement will be effective and binding upon the parties during the entire period of time that my pet is under the care and supervision of Awesome Pets LLC.
